Postal Vote Application Form for an Appointed Proxy
If you cannot download a form you can request one to be sent out to you by completing the attached online form.
If you live in the Dunblane and Bridge of Allan Ward
If you are now unable to attend the polling station, you may be eligible to apply for an Emergency Proxy for one of the following reasons which you were not aware of before 5pm on the 8th March.

You are away for work – Download Form

You are a Scottish prisoner serving a sentence of 12 months or less – Download Form
If you have had a medical emergency and cannot attend the polling station, that took place after 5pm on 8th March or the medical emergency took place before then, but you couldn’t reasonably make an application before this date, you can apply for a medical emergency proxy.

If you have a medical emergency – Download Form
An elector may be able to change their appointed proxy if the circumstances of the appointed proxy on the day of the poll, will or are likely to be such that they cannot reasonably be expected to vote at the polling station.

Change of Proxy Form (must be with the Electoral Registration Officer by 5pm on Thursday 16th March) – Download Form
All forms must be returned to the Electoral Registration Officer before 5pm on Thursday 16th March 2023.
